Club Record Claim Form
Click on the link below to download the Club Record Claim Form. This form should be used if you wish to claim a club record and should be accompanied by a Target Day Score Sheet or a copy of the Tournaments Results Sheet. If you wish to use scores achieved in open competition then a copy of the Results Sheet will suffice.
Hand in the completed form to Tom and he will check the validity of the claim and upload it to the website.
